Cliniko pricing is set by one number: how many practitioners use the account. Checked from the UK on 19 October 2026, Cliniko costs £29 a month for one practitioner and goes up to £259 a month for 26 to 200, in six bands. Reception and admin users are free, every feature is on every plan, and the only usage cost on top is SMS at 5p a message.

That is the short answer. The rest of this page works out what a UK clinic actually pays in a month and a year: cost per practitioner at different team sizes, what reminders add, what annual billing saves, and the things no Cliniko plan covers.

Cliniko pricing in the UK at a glance

These are the six bands on Cliniko's pricing page, shown in pounds to UK visitors, as of October 2026. Cliniko has no feature tiers. The band you sit in depends only on how many practitioners are in the account.

PractitionersPer monthPer year, billed monthlyPer year, billed annually
1£29£348£319
2 to 5£59£708£649
6 to 8£89£1,068£979
9 to 12£129£1,548£1,419
13 to 25£199£2,388£2,189
26 to 200£259£3,108£2,849

The annual column is the monthly price times 11, because paying yearly gives you one month free every year. The page says the prices are in British pounds. It doesn't say whether VAT is included, so check VAT on Cliniko's page or with their team before you budget.

Cliniko's pricing page seen from the UK on 19 October 2026, showing six bands from £29 to £259 a month and SMS credits at 5p per message

Cliniko says its prices haven't changed since it started in June 2011. That is a claim about Cliniko's own prices, not a promise about the future, so re-check the page before you sign up.

Pounds or dollars?

Several articles about Cliniko's cost quote only US dollars, and one says there is no pound price at all. From a UK connection, the page shows pounds and states "Prices are in British pounds (GBP)". When we loaded the same page from outside the UK, it showed US$45 to US$395. If you are in the UK and see dollars, you are probably on a VPN or a non-UK connection.

How much is Cliniko per month for your clinic size?

Because the bands are flat, the cost per practitioner swings a lot depending on where you sit in a band. A clinic with two practitioners pays the same £59 as a clinic with five.

PractitionersBandMonthly priceCost per practitioner
11£29£29.00
22 to 5£59£29.50
32 to 5£59£19.67
52 to 5£59£11.80
66 to 8£89£14.83
86 to 8£89£11.13
109 to 12£129£12.90
129 to 12£129£10.75
1513 to 25£199£13.27
2513 to 25£199£7.96
3026 to 200£259£8.63

Two things follow from this. Hiring a sixth practitioner moves you from £59 to £89, a £30 jump for one person. And a solo practitioner who takes on an associate doubles the bill (£29 to £59) while the cost per head stays near £29 until a third person joins.

Cliniko only counts practitioners. Receptionists, practice managers and other admin users cost nothing, and you can add and remove users at any time. If you change band halfway through a billing period, Cliniko credits the unused part of the old plan and charges the new one pro rata.

What every Cliniko plan includes

The pricing page lists the same inclusions for every band:

  • Every Cliniko feature
  • Free support
  • Unlimited admin and reception users
  • Unlimited locations, patients and file storage
  • 24/7 systems and security monitoring
  • Software updates and constant backups
  • At least 2% of subscriptions donated to charity

Cliniko's FAQ adds that it has never charged extra for a feature, apart from optional SMS. Telehealth, online bookings and treatment notes are part of the subscription. One catch during the trial: some features, telehealth among them, only unlock once a card is on file. Cliniko says this is to stop abuse of free accounts.

Extra costs: SMS, connected apps and phones

Cliniko lists three possible charges in total: the monthly subscription, prepaid SMS credits if you use SMS, and the fees of any third-party apps you connect. Here is what each one means in practice.

Cliniko SMS cost

SMS is prepaid. One credit costs 5p in the UK and covers one message of up to 160 characters. Cliniko's help centre says credits are bought in lots of 200 or more, and that a reminder over 160 characters costs an extra credit. The reminder editor shows an estimate of how many credits each reminder will use, so you can trim the template before it doubles your bill.

SMS messages a month1 credit each (up to 160 characters)2 credits each (over 160 characters)
100£5.00£10.00
250£12.50£25.00
500£25.00£50.00
1,000£50.00£100.00
2,000£100.00£200.00

SMS isn't only reminders. The same credits pay for one-off messages, group messages, follow-ups, form links and practitioner alerts for new online bookings, so count those too.

Connected apps

Cliniko connects with other tools through its connected apps directory, including Xero for accounting, AI note-takers, phone systems and AI receptionists. Cliniko's pricing FAQ says these "may have their own additional fees", and those fees are set by each app, not by Cliniko. Check the app's own price before you count it in.

Phones: the cost no plan covers

None of the bands include a phone line or anyone to answer it. Cliniko runs your diary, notes and invoices. When a patient calls during a treatment or after 6pm, that call still lands on your reception desk, your mobile or voicemail.

For many clinics this is the biggest cost that never shows up in a Cliniko price comparison: the new patient who rings once, gets voicemail and books somewhere else. The usual fixes are a receptionist's hours, a human answering service or an AI receptionist. Chimehill is an AI receptionist that answers your clinic's calls and books, moves and cancels appointments directly in Cliniko, and it only tells a caller they're booked once Cliniko has confirmed the save. You can see how it works with Cliniko, or compare plans priced by calls a month.

Real monthly totals for three UK clinics

Putting the subscription and SMS together gives a truer monthly figure. These are worked examples, not averages: we picked plausible reminder volumes and assumed one credit per message.

ClinicBandSMS a monthMonthly totalYearly, billed annually
Solo physio1 practitioner, £29150 at 5p = £7.50£36.50£409
Four-practitioner clinic2 to 5, £59600 at 5p = £30£89.00£1,009
Eight-practitioner clinic6 to 8, £891,200 at 5p = £60£149.00£1,699

The yearly figure is the annual subscription (11 months) plus 12 months of SMS. Add any connected-app fees on top, and check how VAT applies to your account.

Monthly vs annual billing

You can switch between monthly and annual billing from Settings once you have an account. Annual billing gives you one month free every year, on top of the trial. That saves between £29 (one practitioner) and £259 (26 or more) a year.

There is no minimum contract either way. You can cancel at any time and won't be billed again. Payment is by credit or debit card, including Visa and Mastercard.

Cliniko free trial

The standard Cliniko free trial lasts 30 days and doesn't need a card. Everything you set up during the trial stays in the account if you carry on.

  • No card on file at the end: the account closes on its own and you don't need to cancel.
  • Card on file: you are billed automatically when the trial ends. Cliniko emails a reminder five days before.
  • If you leave after the trial, Cliniko says your data is kept for 12 months after the cancellation date.

CSP members' 90-day trial

The Chartered Society of Physiotherapy describes Cliniko as the only practice management platform it officially partners with, and its partner page mentions a special 90-day free trial for CSP members. That page was last reviewed in December 2021, so confirm the current offer with Cliniko or the CSP before you rely on it.

Discounts, charities and contracts

Cliniko doesn't offer discounts. Its pricing FAQ says everyone pays the price on the website. The exception is bigger than a discount: Cliniko is free for registered charities and educational institutions, set up on request through Cliniko's support team.

Apart from that, annual billing is the only way to pay less. There are no setup fees or upfront costs.

Is switching your data to Cliniko free?

Yes, for a list of supported systems. Cliniko's team runs a managed data transfer from many other practice systems at no charge, but you need to book it in advance. For systems not on that list, you can import patients and other records yourself from a CSV file. Cliniko suggests re-creating upcoming appointments and outstanding balances by hand, and attaching older treatment notes as files.

Ask early what will and won't move from your current system. The software may be free to switch to, but the staff time spent checking imported records is not.

Is Cliniko good value?

On price alone, Cliniko is easy to budget for. The bands are public, every feature is included, admin users are free and there's no contract. At the top end of a band (five, eight or 12 practitioners), the cost per head is about £11 a month or less.

Where it costs more than it looks is at the bottom of each band (two, six, nine or 13 practitioners) and in SMS for clinics that send a lot of reminders. Price is also only part of value. Our Cliniko review covers what users like and where it falls short, and if you're still choosing a system, our guide to physiotherapy practice management software compares the UK options in pounds.

Questions clinics ask

Is Cliniko priced per practitioner or per clinic?

Per account, by band. You pay one monthly price based on how many practitioners use the account, not a fee for each one. Two practitioners and five practitioners both pay £59 a month in the UK. Locations are unlimited on every band.

How much do Cliniko receptionist licences cost?

Nothing. Receptionist and other admin user licences are free and unlimited on every plan. Only practitioners count towards your band, so you can add front-desk staff without changing the price.

Does Cliniko have hidden fees?

Cliniko lists three charges: the subscription, prepaid SMS at 5p a message, and the fees of any third-party apps you connect. There are no setup fees or paid feature add-ons. The one thing to check yourself is how VAT applies to your account.

Does Cliniko charge you automatically after the free trial?

Only if you entered a card. With a card on file, billing starts when the 30 days end and Cliniko emails you five days before. Without a card, the account closes by itself.

How much does Cliniko cost per year?

Billed annually, Cliniko costs £319 a year for one practitioner, £649 for two to five, £979 for six to eight, £1,419 for nine to 12, £2,189 for 13 to 25 and £2,849 for 26 to 200. That is 11 months' price, because annual billing includes one month free. Add SMS on top.

Is Cliniko free for charities?

Yes. Cliniko is free for registered charities and educational institutions. You contact Cliniko's support team to have a free subscription set up. Other clinics pay the listed price, as Cliniko doesn't give discounts.

Does Cliniko charge to transfer data from another system?

No. Cliniko's managed transfer from supported systems is free, booked in advance. For other systems you can import CSV files yourself, with help from Cliniko's support team.

What isn't included in any Cliniko plan?

SMS credits, third-party app fees, and anything to do with your phones. Cliniko doesn't answer calls, so missed calls during treatments and after hours need a separate answer, such as a receptionist, an answering service or an AI receptionist for clinics.

Do Cliniko's prices include VAT?

The pricing page says prices are in British pounds but doesn't say whether VAT is included. Ask Cliniko's support team or check your first invoice before you set a budget, especially if your clinic is VAT-registered.
